Patient Information

Your First Visit

Core Wellness and Physical Therapy, serving residents of Milford, Loveland, Goshen, Mariemont, and Anderson, offers a full range of physical therapy and wellness services. Our services range from pelvic floor PT to dry needling and a dedication to help you recover fully and lead your best, active life.

During your first visit, you can expect to receive a thorough assessment of your injury or condition and we will discuss with you our recommended program of treatment.

Under normal conditions, you will also receive some physical therapy at this time as well, so be sure to wear comfortable clothing that allows you to move easily. If you are coming because of an injury, wear something that allows the physical therapist to access the injured spot.

Please bring with you any notes from your doctor if you are already seeing one. We would also appreciate a list of any prescribed or over-the-counter medications you take regularly.

Our visits last approximately an hour and for each visit, you will be seeing a Doctor of Physical Therapy. Our goal is to help you heal and achieve your personal movement and strength goals.

Insurance

To give our clients the intensive, hour-long appointments that we know promote less visits and faster recovery time overall, we operate outside of any insurance network. Clients will be asked to settle their accounts at each visit and we will provide itemized receipts for those who wish to submit it to their insurance providers.

Medicare Beneficiaries: Because we are not Participating Medicare Providers, we can only accept Medicare beneficiaries as patients when the patient does not want Medicare billed for any PT services. The request to not involve Medicare must be made up front by the patient and be made of the patients own free will.

The only way we can provide you with PT if you carry Medicare insurance is when you truly don't want Medicare involved and you ask us up front that Medicare not be billed or involved in your care. If you do want to use your Medicare benefits, we cannot provide you with treatment at our clinic, but can help you find a Medicare provider.

Good Faith Estimate: You have the right to receive a "Good Faith Estimate" explaining how much your medical care will cost. Under the new law that began Jan 1, 2022, health care providers need to give patients who don't have insurance or who are not using insurance an estimate of the bill for medical items and services.

You have the right to receive a Good Faith Estimate for the expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

You have the right to ask for a Good Faith Estimate before you schedule an item or service. Please visit www.cms.gov/nosurprises for more information.

I went to see Kristin so I could get my body back in check. After gall bladder surgery, I felt off and my husband and I were also trying to conceive, but having difficulty. Kristin was so personable, yet very professional. She evaluated me and then worked with me in a variety of ways: myofascial release, acupressure, and healing touch. After only a couple of sessions, my body feels better …and we’re expecting! I highly recommend Kristin! Thank-you!

I want to excitedly share with you that I ran a 10.8 mile trail race this morning. Barely any pain during the run or now, several hours later. I want to thank you again for helping me so very much :)

I came to Kristin a couple months ago because I was having a lot of knee pain. From the very first visit, I could tell she was extremely knowledgeable and patient-friendly! She asked questions and had me do various movement tests and then she did some hands-on work. I really felt like she was concerned with treating the whole person, and I left her office feeling significantly better and armed with a very manageable list of exercises to do. Since then, I've been back a few more times and she's altered the exercises as I've gotten stronger. Now, I feel like my old self and am back to running again! Thanks, Kristin!

In the summer of 2009, I suffered a very complex and life-altering injury. When I was finally able to start physical therapy, I began a search to find a physical therapist who could help me. After months of searching, meeting with many PTs, and seeing my hope for improvement slipping away, I was fortunate to be referred to Kristin Thomas, DPT. By the end of my first session with Kristin, I knew that I had found the right person to assist me on my path to recovery. Whereas other PTs were baffled by my complex conditions and unsure of how to help, Kristen’s unique combination of orthopedic and manual physical therapy enabled me to regain strength, flexibility, and range of motion. Most importantly, Kristin’s treatments reduced my chronic pain significantly and as a result gave me something I thought was gone forever – hope. As an added bonus, Kristin was also able to help me with several musculoskeletal injuries I had sustained from years of playing competitive sports. For anyone in need of an incredibly talented, compassionate and highly qualified physical therapist, I give Kristin Thomas my highest recommendation.
